ALL VOLUNTEERS must complete the Volunteer Registration Form, Code of Conduct Form (links listed below) along with a copy of your current California driver's license and return everything to the Office Manager, Irene Nelson. You may also email her the completed information at inelson@conejousd.org

The TB Assesement is valid for four years. If you discover that you are not current on your TB Assessment form, please complete the form along with an Authorized Health Care Providers Certification and return to Mrs. Nelson at your earliest convenience.   

PLEASE NOTE: We will not be able to process your volunteer forms unless all the necessary information is provided. Your teacher will notify you once you have been cleared. Also, please remember to always bring your driver's license with you to check in the Front Office. Thank you for volunteering!  We appreciate you!
