• Registration Information

    If your child is new to the Conejo Valley Unified School District, you will submit your registration documents to your neighborhood school. If you think Maple may be your neighborhood school, you can look up your residential address on the Maple Street Index.

    If you're unsure what your neighborhood school is, you can use the District Street Index to look it up by your residential address.

    If you wish your child to attend Maple, but you have a different neighborhood school, you may submit a School Choice Application to CVUSD Instructional Services to request the transfer to Maple. The school choice application period begins November 1st prior to the applicable school year.     Registration Requirements: (These residency documents are required for each change of address/residence).

    Proof of Birth:  A legal certified copy of your child’s birth certificate, baptismal record, or passport (hospital certificates are not acceptable).  To enter Kindergarten, your child must turn 5 years old on or before September 1st. 

    Proof of Identity: A valid Driver’s License or ID card

    Proof of Residency: 

    Two (2) different CURRENT utility bills (i.e. gas, water, trash, or electric) showing both a Service Address and Mailing Address

         a. Telephone bills may not be used (because they don’t show a Service Address)

         b. If applicable, mobile home owners submit monthly HOA itemized statement showing utilities

    Additional Documents Required:

          1.  Families purchasing a residence in CVUSD attendance boundaries and will occupy the residence within 90 days:

               a. Executed Escrow Papers for residential domicile, including closing settlement date

               b. Within three weeks of move-in, two utility bills showing both service address and mailing address

          2.  Families renting a residence in CVUSD attendance boundaries and will occupy the residence within one week:

               a. Has/will have utility service in their own name:

               b. Executed Rental Agreement (original)

          3. Two CURRENT utility bills showing both service address and mailing address. (If new resident, must supply two utility bills within three weeks)

               1. Does not/will not have utility service in their own name?

               2. Executed Rental Agreement (original)

         4. Completion of Form AV – Address Verification Form (CVUSD 98-00160)

         5. Two CURRENT utility bills from your landlord

         6. Student living with someone other than parent/guardian (adult sibling, relative, grandparents, etc.):

               a. Completion of two page “Caregiver’s Authorization Affidavit” (CVUSD Form 98-00111), notarized and approval signature from Student Support Services.

         7.  Families living with others (renting a room, living with family members or friends, etc.), without a formal rental agreement or utility service in their name:

               a. Completion of Form AV – Address Verification Form (CVUSD Form 98-00160)

               b. Two CURRENT utility bills from your landlord

    Measles, Mumps, and Rubella (MMR or MMR-V)

    2 doses

    (Both given on or after 1st birthday.  Only one dose of mumps and rubella vaccines are required if given separately.)

    1 dose

    (Dose given on or after 1st birthday.  Mumps vaccine is not required if given separately.)

    2 doses of MMR or any measles-containing vaccine

    (Both doses given on or after 1st birthday.)

    Hepatitis B

    (Hep B or HBV)

    1 dose for ages 7-12 years.

    2 doses for ages 13-17 years.

     

    *New admissions to 7th grade should also meet the requirements for ages 7-17 years.

    California Kindergarten Readiness Act

    These minimum age requirements are set by California law, under the
    California Kindergarten Readiness Act of 2010. Neither Maple nor CVUSD can
    make any exceptions to these requirements. Please visit the
